Satisfaction scores on a scale of 1 (highly dissatisfied) to 4 (highly satisfied). Importance scores on a scale of 1 (not important at all) to 4 (very important).

- Dentist Visits in Past year (0, 1, 2, 3, 4, 5, 6 or more times)
- Dental treatments received and intend to receive:
- Preventative dental care (cleaning, hygiene, x-rays)
- Basic dental care (fillings, crowns, bridges, replacements)
- Major dental work (braces, dentures, implants, surgery)
- Cosmetic dental work (whitening and laminates)
- Dentures (y/n)
- Personal Relationship with Dentist from 1 (no personal relationship, I see who is available) to 5 (very close, on a first name basis, known for a long time)
- Demographics: age, ethnicity, gender, family size, household designation, home ownership, income, education, employment, and education.
